To change your name with APSU, please submit a photo ID and an official government document -example - Social Security card, marriage certificate, or other court order via email to admissions@apsu.edu.  The documents and request can also be mailed to Austin Peay State University, Office of Admissions, Box 4548, Clarksville, TN 37044.

If your major is Undecided, please select a concentration from the list under Academic Focus Concentrations.
