Graduate Assistant Forms

- GTA Self Evaluation
This form is to be completed every semester by Graduate Teaching Assistants (GTA's). - Form for Scholarship Award in Addition to GA Scholarship
In some cases, graduate assistants are awarded scholarships in addition to their assistantships. If you receive an additional scholarship, please submit this form to the Dean of the College of Graduate Studies upon acceptance of the award. - Graduate Assistant Request to Add/Reduce Course Hours/Request for Undergraduate Course
Hours to be Covered by Assistantship
The minimum number of credit hours which may be taken in a semester by a graduate assistant is six (6) and the maximum is ten (10). Permission from the Dean of the College of Graduate Studies is required if the credit load is fewer than six (6) or more than ten (10) hours. This form is also used to request that an assistantship cover undergraduate course work that is required or related to the program of study. Please complete this request PRIOR to the beginning of the each semester. Failure to submit the request may result in outstanding fees being placed on your student account. - Graduate Assistant Summer Semester Out of State Fee Waiver
Graduate Assistants who are classified as out-of-state residents are eligible for summer fee waivers offering in-state rates for summer courses. Submit the request form to the College of Graduate Studies for approval and processing. - Request for Additional Employment
Students appointed to graduate assistantships may be permitted additional on-campus employment under special circumstances. Permission to work additional hours is granted solely by the Dean of the College of Graduate Studies and is not guaranteed. For domestic students, the total ombined hours of APSU employment may not exceed 29 hours per week. For students with NRA status, the total combined horus of APSU employment may not exceed 20 hours per week. - Suggested Time Reporting Sheet
This form is provided as an example of how to keep track of the hours worked for personal use. Please check with your department supervisor to verify the method used for tracking the hours you work.
