Appeal Forms

• Tennessee Scholarship Appeal/ Leave of Absence Request Form - The HOPE Lottery Scholarship, TN Promise, and TN Reconnect is awarded based on policies set forth by the Tennessee Student Assistance Corporation (TSAC). The policies for these awards allow an appeal process for students who fail to meet initial or continuing eligibility.
• Scholarship Appeal Form - This form is used to appeal the loss of an academic scholarship. Normally, a student is not able to appeal the loss of the scholarship until after one semester/term has elapsed. If extenuating circumstances exist, an appeal may be submitted to the Office of Student Financial Aid before the lapse of one semester/term.

 Satisfactory Academic Progress Appeal Form

• Satisfactory Academic Progress Appeal Form - This form must be completed when a student has been notified by the Student Financial Aid Office that they are in violation of the Satisfactory Academic Progress Policy Guidelines.

• Academic Review Form - This form must be completed by the student and the student’s advisor if the individual is in violation of maximum timeframe or pace standards.
